Primary Care Zone | Primary care homes – advanced primary care networks: impact and lessons learnt

NAPC are the architects of the primary care home (PCH) model – the original primary care network which NHS England Chief Executive Simon Stevens launched in 2015 and went on to inform the development of national PCN policy. There are now more than 230 PCHs across England and many more emerging as part of NAPC’s PCN development work with STPs and ICSs.

Primary care home leaders will outline the difference the model has made to staff, patients and the wider health system, how they overcame the challenges and offer advice for new emerging primary care networks. The panel will also examine the future as set out in the  NHS Long Term Plan and the opportunities for greater collaboration with community pharmacy and other primary care services.
