Traditional ways of delivering health and care services are outdated, with more collaborative approaches required to redesign care and improve population health. The NHS Long Term Plan sets us on a course for greater integration, including the goal for all parts of the country to be part of an integrated care system by 2021. The ambition is right but delivering this vision will require major changes. In this session, we hear from colleagues across the UK about what they see as the key ingredients required to deliver on the Long Term Plan’s ambitions.
