How do you come together as a whole system, and at all levels, to redesign how care is provided to older people?

The Birmingham health and social care economy have come together to take a whole system approach to reimagine and redesign how we deliver joined up support for older people. We want our city to be a great place to grow old in, while improving service quality and productivity, and achieving measurable financial savings.  

We will share the journey we have been on as system leaders to agree our joint vision. Then, we’ll focus on the bottom-up approach we are taking to this large scale and complex transformation programme, and some of the challenges faced – for example changing long standing operational and behavioural practices and redesigning how we commission and deliver services.
