Primary Care
How does primary care prioritise need at neighbourhood and place level when it is at odds with system-wide priorities?
This session will examine how a primary care network may have different population health priorities for their patients and how they meet and support these challenges when they must also deliver the population health requirements that are set at system level.
This session forms part of the Primary Care feature zone that is supported by Roche Diabetes Care.
